About the position

The Hospice Social Worker at Angels Care Hospice is responsible for providing medical social work services to patients and their families during terminal illness. This role involves assessing psychosocial needs, providing counseling, and coordinating with interdisciplinary teams to ensure comprehensive care. The social worker will also maintain clinical records and serve as a liaison between patients, families, and community resources.

Responsibilities

  • Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment.
  • Provide assessments of the patient's residence and assist when it is unsafe, developing alternative plans as needed.
  • Conduct social evaluations, including family dynamics, caregiver abilities, and communication patterns, and plan interventions accordingly.
  • Counsel patients, families, and caregivers regarding stress and coping difficulties, providing crisis intervention when necessary.
  • Educate the interdisciplinary team on special needs related to the culture of the patient and family.
  • Maintain clinical records on all patients referred to social work, preparing clinical and progress notes.
  • Provide information and referral services for patients, families, and caregivers regarding practical and environmental needs.
  • Serve as a liaison between patients, families, caregivers, and community agencies.
  • Maintain collaborative relationships with organization personnel to support patient care.
  • Develop contracts with public and private agencies as resources for patients and personnel.
  • Actively participate in quality assessment performance improvement teams and activities.

Requirements

  • Master of Social Work (MSW) degree is required.
  • Experience in medical social work, particularly in hospice or palliative care settings.

Nice-to-haves

  • Experience with crisis intervention and counseling in a healthcare setting.
  • Familiarity with community resources and agencies related to patient care.
