About the position

Capital City Hospice is seeking a dedicated Social Worker to perform psychosocial assessments for patients and families in the hospice program. The role focuses on ensuring that the needs of patients and their families are met through comprehensive care plans, while also providing support and resources to assist in their journey. The position emphasizes exceptional service, teamwork, and a commitment to the well-being of both patients and staff.

Responsibilities

  • Perform psychosocial assessments of patients and families/caregivers in a timely manner.
  • Ensure identified needs are addressed in the patient's plan of care.
  • Provide assessments in the patient's identified residence and assist when safety is a concern.
  • Demonstrate exceptional service to patients, families, referral sources, co-workers, and other customers.
  • Assist team members as needed to ensure consistent service delivery.
  • Counsel patients and families/caregivers regarding stress and coping difficulties.
  • Provide accurate and timely documentation of patient services reflecting the plan of care.
  • Assist with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver.
  • Obtain resources from community social, health, pharmaceutical, and welfare agencies to meet patient/family needs.
  • Serve as a liaison between patients, families/caregivers, and community agencies.

Requirements

  • Bachelor's degree in Social Work; Master's degree preferred.
  • Current State Licensure in Social Work appropriate to the state of practice.
  • Minimum of one year's experience in health care or hospice setting.
  • Valid driver's license and proof of insurance.

Nice-to-haves

  • Experience in hospice or palliative care settings.
  • Knowledge of community resources and support services.
