About the position

The Hospice RN Case Manager is responsible for planning, organizing, and directing hospice care for patients and their families. This role emphasizes community health education and requires the nurse to assess patient needs, develop care plans, and provide professional nursing care in a home setting. The position involves working closely with patients and families to ensure their needs are met effectively and compassionately.

Responsibilities

  • Completes initial assessments of patients and families to determine hospice needs.
  • Provides a complete physical assessment and history of current and previous illnesses.
  • Utilizes all elements of the nursing process to provide professional nursing care.
  • Assesses and evaluates patient's status by writing and initiating a plan of care.
  • Regularly evaluates patient and family/caregiver needs.
  • Uses health assessment data to determine nursing diagnosis.
  • Develops a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions.
  • Includes the patient and family in the planning process.
  • Provides health care instructions to the patient as appropriate per assessment and plan.
  • Participates in on-call duties as defined by the on-call policy.
  • Ensures that arrangements for equipment and other necessary items and services are available.
  • Supervises ancillary personnel and delegates responsibilities when required.

Requirements

  • At least 1 year experience in Hospice within the last 3 years.
  • Current California RN license.
  • Excellent written and verbal communication skills.
  • Valid CA Driver's License and auto insurance.
  • Current CPR certification.
  • Current TB/Physical clearance.
  • Pass background clearance.

Nice-to-haves

  • Bilingual (English/Spanish).
  • Baccalaureate degree in nursing.
