The Registered Nurse will provide in-home services to our patients. This role involves providing services that require substantial and specialized nursing skill, in accordance with the plan of care. Caseloads will vary from approximately 12-18 patients, depending on factors such as the complexity of patient/family needs, geography, and whether patients are home-based or facility-based. The nurse will demonstrate competency in various skills including pain management, symptom control, infusion therapy, infusion therapy device care, wound care, and aseptic technique. Critical thinking, analysis, and clinical judgment are essential in caring for patients and families facing terminal illness. The role requires addressing the physical, emotional, psychosocial, and spiritual needs of the patient and their family/caregiver, making referrals to other disciplines as needed, and coordinating the total Plan of Care to ensure continuity of patient care by liaising with other health professionals. The Hospice Registered Nurse acts as a Case Manager, ensuring equitable and beneficial utilization of resources to meet patient needs, managing care from admission through discharge, and coordinating with the interdisciplinary team on bereavement.
