About the position
The Host Home/Shared Living provider role with Mosaic at Home® offers a meaningful opportunity to support individuals with disabilities in a home environment. Providers will share their home with a person with a disability, ensuring a safe and healthy living space while assisting with daily living tasks and fostering independence. This position allows for a flexible income based on the level of support provided, with an average annual income of $33,000, which is federally tax-free, plus room and board.
Responsibilities
- Provide support for tasks such as cooking, cleaning, laundry, budgeting, and decision-making.
- Assist with skill acquisition and medication administration.
- Accompany individuals to medical appointments and support their overall health and well-being.
- Document daily activities and support via a web-based case management application (THERAP).
- Facilitate fun and leisure experiences, promoting community inclusion and socialization.
- Ensure compliance with all state and agency policies and procedures.
- Communicate effectively with team members as needed.
- Support the physical needs of the individual served, including transportation.
Requirements
- Must be a resident of Illinois and own or rent a home.
- Must be at least 21 years of age.
- Reliable computer and internet access is required.
- Ability to pass State/Federal and APS/CPS background checks.
Nice-to-haves
- Experience in caregiving or support roles is preferred but not required.
- Ability to provide assistance with bathing, toileting, feeding, and behavioral support.
