About The Position

Warm Hearth Village, a nonprofit community, is seeking a compassionate and skilled Speech Language Pathologist (SLP) to join our home health team on a PRN basis. The SLP will evaluate and treat patients in their homes to improve communication, cognition, swallowing, and overall functional independence. This role offers flexibility, autonomy, and the opportunity to make a meaningful impact by helping patients achieve their highest level of function and quality of life in the home setting.

Requirements

Master’s degree in Speech Language Pathology from an accredited program
Current Speech Language Pathologist license in the state of Virginia
Current CPR certification
Valid driver’s license, reliable transportation, and auto insurance
Strong communication, organizational, and clinical skills
Ability to work independently and manage time effectively

Nice To Haves

Certificate of Clinical Competence (CCC-SLP) preferred
Home health experience preferred, but not required

Responsibilities

Perform comprehensive speech therapy evaluations and develop individualized treatment plans
Provide skilled therapy services for speech, language, cognitive, voice, and swallowing disorders
Assess and treat patients with dysphagia and provide dietary and safety recommendations as appropriate
Educate patients and caregivers on communication strategies, swallowing precautions, cognitive support, and home exercise programs
Monitor patient progress and adjust treatment plans as needed
Complete accurate and timely clinical documentation in the electronic medical record
Communicate effectively with physicians, nurses, therapists, and interdisciplinary team members
Participate in care coordination and case conferences as needed
Ensure compliance with Medicare, state, and agency regulations
Travel to patient homes within assigned territory
