We are seeking a compassionate, organized, and detail-oriented Scheduler to join our Home Health team.
Requirements
- High school diploma or GED required, Associate degree preferred
- Minimum of 1 year of practical work experience
- Strong communication, organization, and problem-solving skills
- Ability to manage multiple priorities in a fast-paced environment
- Ability to make decisions and adapt quickly with changing information
- Intermediate Microsoft Office skills
- Ability to learn systems such as HomeCare HomeBase and Workday
Nice To Haves
- Previous home health scheduling experience preferred
- Understanding of patient care coordination and scheduling workflows preferred
- Bilingual Spanish/English preferred
Responsibilities
- Coordinate and schedule patient visits for clinical staff
- Manage clinician schedules to ensure timely and efficient patient care coverage
- Support key workflows throughout the patient care journey
- Communicate with Clinical Team Managers, field clinicians, physicians, patients, and facility staff
- Adjust schedules and staffing plans based on changing patient and operational needs
- Monitor scheduling productivity and support branch operational goals
- Respond quickly to scheduling conflicts and unexpected changes
- Maintain accurate scheduling documentation and communication
- Support patient satisfaction through proactive communication and coordination
