About the position

The Home Health Physical Therapist (PT) role at Touchmark on South Hill involves providing personalized physical therapy services to patients in their homes throughout Spokane County. This part-time position offers the opportunity to positively impact the lives of patients and their families while working independently and collaboratively with an interdisciplinary team.

Responsibilities

• Review physician referrals and create personalized physical therapy treatment plans.
• Record and document patient condition, education, and physical progress.
• Work with an interdisciplinary team to develop a plan of care.
• Create and monitor PT treatment plans for physical therapy assistants.
• Work independently in patient homes using good judgement and critical thinking skills.
• Transport to patient homes throughout Spokane County.

Requirements

• Current Physical Therapist (PT) License in good standing in Washington State.
• Current CPR & First Aid certifications.
• Transportation to patient homes, current driver's license, and vehicle insurance.
