About the position

Join our team as a Home Health Physical Therapy Assistant (PTA) where you will provide compassionate, high-quality care to patients in their homes. This role allows you to make a meaningful impact on patients' lives by delivering exceptional therapy services, collaborating with a dedicated interdisciplinary team to promote wellness, manage chronic conditions, and improve health outcomes.

Responsibilities

  • Implement care plans designed by a licensed Physical Therapist to improve patient mobility and independence.
  • Deliver therapeutic exercises and treatments tailored to individual patient needs.
  • Educate patients and caregivers on therapy techniques to support recovery and ensure safety.
  • Maintain accurate documentation of treatments and progress to ensure effective care.
  • Work collaboratively with an interdisciplinary team to deliver holistic, patient-centered care.

Requirements

  • At least 9 months as a Physical Therapy Assistant (home health or community care experience preferred).
  • Associate's degree in Physical Therapy Assisting from a CAPTE-accredited program.
  • Active Physical Therapy Assistant license in the state of agency operation.
  • Current CPR certification.
  • Valid driver's license and reliable transportation for patient visits.
