About The Position

Navitas Healthcare, LLC is seeking an experienced Home Health Occupational Therapist (OT) for a contract assignment based in Hood River, Oregon. This opportunity offers a consistent weekday schedule, one-on-one patient care, and no weekend or on-call responsibilities. Both experienced travelers and first-time travel therapists are encouraged to apply. Local candidates are also welcome. Common diagnoses include: Stroke Recovery, Orthopedic Post-Operative Rehabilitation, Dementia, Parkinson’s Disease, Multiple Sclerosis (MS), Amyotrophic Lateral Sclerosis (ALS), and Other Acute and Chronic Neurological Conditions.

Requirements

Active Oregon Occupational Therapist (OT) License.
Active Washington Occupational Therapist (OT) License.
Minimum 1 year of Home Health Occupational Therapy experience.
Direct OASIS documentation experience required.
Current BCLS Certification (AHA).
Valid Driver's License.
Clean Driving Record.
Reliable Transportation.

Responsibilities

Provide comprehensive occupational therapy evaluations and treatment services in patients' homes.
Develop individualized treatment plans focused on improving independence and daily living skills.
Perform functional assessments and home safety evaluations.
Complete OASIS assessments and documentation accurately and timely.
Educate patients and caregivers on adaptive equipment, home modifications, and therapeutic exercises.
Utilize Epic EMR and mobile documentation tools for patient charting.
Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams.
Monitor patient progress and adjust treatment plans as appropriate.
Ensure compliance with home health regulations, safety standards, and agency policies.
