About the position
Ameri Hospice and Home Health Care is seeking a Registered Nurse (RN) and Licensed Vocational Nurse (LVN) to provide hospice care in Los Angeles County. The role involves planning, organizing, and directing hospice services while collaborating with healthcare professionals to meet the needs of patients and their families in their homes and communities.
Responsibilities
- Coordinating the care of the patients assigned to you.
- Making regular visits to patients' homes to assess patients' physical, emotional, spiritual, and social needs.
- Working collaboratively with the hospice physician and your supporting LVNs to carry out appropriate interventions.
- Helping the agency to continuously increase the quality of care.
Requirements
- Licensed as a Registered Nurse (RN) or Licensed Vocational Nurse (LVN).
- Valid Driver's License and reliable vehicle with current insurance.
- Minimum of two years' experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred.
- CPR Certified.
