About the position

The Home Health Aide at Custom Home Health Inc. is responsible for providing personalized care to patients in their homes, ensuring their comfort and well-being. This role emphasizes hands-on personal care, collaboration with nursing staff, and adherence to care plans, all within a supportive and flexible work environment.

Responsibilities

• Providing hands-on personal care including baths, back rubs, and oral hygiene.
• Changing bed linen and assisting patients with dressing and undressing.
• Performing skin care to prevent breakdown and assisting with toileting activities.
• Keeping the patient's living area clean and orderly as appropriate.
• Planning and preparing nutritious meals and assisting in feeding the patient if necessary.
• Following the personalized plan of care in collaboration with the nurse and interdisciplinary team.
• Assisting in ambulation and exercise according to the plan of care.
• Doing the patient's laundry as appropriate.
• Reporting on the patient's condition and significant changes to the assigned nurse.
• Adhering to the organization's documentation and care procedures and standards of personal and professional conduct.
• Participating in orientation, in-service programs, staff meetings, case conferences, QAPI, and Emergency Preparedness training.

Requirements

• Valid certification through the state (required)
• 1+ years of certified nursing experience
• At least 18 years of age
• Ability to read and follow written instructions and document care given
• Current CPR certification
• Demonstrates tact, patience, and good personal hygiene
