About The Position

As a Certified Home Health Aide (HHA), you will provide care to clients in their home, following their plan of care. You make a positive difference each and every day by demonstrating care and compassion to your patients.

Requirements

  • Compassionate, patient focused and dependable professional
  • Critical thinking skills, decisive judgment and the ability to work with minimal supervision
  • Exceptional documentation and communication skills
  • Ability to read, write and speak English, ability to follow verbal and written instructions
  • Valid NYS Driver’s License
  • HHA Certified or Ability to meet one of the following eligibility requirements for Competency:
  • Currently certified CNA along with completion of written test and one day HHA Skills competency class within 4 days of hire.
  • Documented home health aide or nurse aide training and competency evaluation from an out-of-state training program
  • The Certified HHA who wishes to be employed after a two-year lapse in employment as a HHA must take the 1 Day Skills Competency Class. They must complete a written test and pass the one day HHA Skills competency class within 4 days of hire and before working as an HHA. If the candidate has worked as a Certified HHA within the past 24 months, and is actively certified, no class is needed.
  • Nursing student who has documented evidence of successful completion of course work requiring mastery of home health aide tasks within the past 24 months-Fundamentals of Nursing and must show completion of written test and one day HHA Skills competency class within 4 days of hire.
  • Veteran who was trained in the United States Military as medical technician or medic must show completion of written test and one day HHA Skills Competency Class within 4 days of hire.

Responsibilities

  • Patient Care & Service. Promote and restore patients' health by completing all activities listed in the care plan; collaborate with Community Health Nurse/Therapist; perform various activities such as personal care, light housekeeping, meal preparation and errands
  • Communication. Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularly
  • Documentation. Accurately and completely document client care on activity sheet
  • Compliance. Adhere to required department and system protocols, HIPPA regulations, local, state, federal requirements.
