About the position

The Home Health Aide Nurse Supervisor is responsible for supervising Home Health Aides (HHAs) under the guidance of the Chief Home Health Services Officer. This role involves ensuring compliance with regulatory requirements and maintaining high standards of care for patients receiving home health services. The supervisor will oversee the hiring, training, and orientation of new HHAs, as well as conduct regular home visits to evaluate the services provided. The position requires effective communication and coordination with schedulers to manage HHA assignments and ensure patient needs are met efficiently. Additionally, the supervisor will provide direct patient care as needed and handle various administrative responsibilities related to HHA services.

Responsibilities

  • Responsible for hiring, orientation, training and on-boarding of new HHAs.
  • Supervise and direct all HHA services for PACE patients, including home visit reviews.
  • Conduct initial home visits to introduce HHAs to new patients and the plan of care.
  • Conduct Supervisory Home Visits at regular intervals to patients receiving HHA services.
  • Work with Schedulers to ensure efficient scheduling of HHAs for case assignments.
  • Ensure timely communication of HHA schedule changes and manage case coverage.
  • Monitor the flow of information from HHAs' Daily Activity Sheets into patient records.
  • Schedule, develop and conduct in-service programs and case conferences for HHAs.
  • Conduct certified training programs for potential HHAs as necessary.
  • Provide direct patient care as necessary during home visits.
  • Conduct annual individual HHA performance reviews and skills assessments.
  • Field HHA calls from patient homes and facilitate appropriate responses.
  • Assist in investigating, resolving and documenting complaints and incidents related to HHA services.
  • Act as a back-up to the HHA Services Department Call-Out line.
  • Responsible for staff compliance with billing, personnel policies, patient grievances, and regulatory mandates.
  • Handle an average of 20-25 client visits per week.
  • Handle other responsibilities as assigned by the Chief Home Health Services Department.

Requirements

  • Registered Nurse licensed under the laws of the Commonwealth of Massachusetts required.
  • Two years of nursing experience, with at least 1 year in home health care or long-term care.
  • At least 6 months of experience supervising HHA Services.

Nice-to-haves

  • Bilingual language skills; additional language skills in Spanish, Cape Verdean Creole and/or Portuguese preferred.
