About the position

The Home Health Aide - Hospice position at PeaceHealth involves providing personal care and household tasks for home health and hospice patients. The role includes performing limited nursing care and therapy services under the supervision of a registered nurse, ensuring that patients receive the necessary support as outlined in their care plans.

Responsibilities

• Perform patient care duties/tasks as outlined on the patient specific home health/hospice care plan.
• Perform basic technical skills such as CPR, taking and recording vital signs, changing non-sterile dressings, measuring and recording fluid intake/output, and assisting patients with taking oral medications as directed by the physician or nurse.
• Assist patients with personal care including bathing, mouth and skin care, grooming and dressing, and assistance with bed pans and/or urinals.
• Provide assistance to patients with walking, exercising, and moving in and out of bed, including the use of special equipment like walkers, wheelchairs, or canes.
• Assist with meal preparations and/or feeding, change bed linen, and perform light housekeeping as outlined on the patient's care plan.

Requirements

• A minimum of two years CNA and/or Home Health Aide experience in a nursing home, hospital, or home care setting required.
• Current BLS (Basic Life Support) certification required at date of hire.
• Valid driver's license (OR, WA or AK as applicable) and proof of auto insurance required.

Nice-to-haves

• Home Health experience is preferred or will orient to point of competency.
