About The Position

We are hiring a Home Finder / Foster Care Case Manager to support children and families involved in foster care and adoption. This role combines foster parent recruitment, home studies, and case management services to help children achieve safe, stable, and permanent placements. This is an excellent opportunity for Social Workers, Case Managers, and Human Services professionals who are passionate about child welfare, foster care, and adoption services.

Requirements

Bachelor’s Degree in Social Work, Psychology, Human Services, Counseling, or related field
Minimum 2 years of experience working with children and families
Valid driver’s license and reliable transportation

Nice To Haves

Master’s Degree in Social Work or related field
Social Work License (LGSW, LCSW, or equivalent)
Experience in foster care, adoption, child welfare, or case management

Responsibilities

Provide case management services to children and foster/adoptive families
Conduct regular home visits, school visits, and monthly face-to-face contacts
Monitor child safety, well-being, and permanency progress
Attend MDT meetings, court hearings, and treatment team meetings
Maintain documentation in compliance with Medicaid, state, and agency requirements
Recruit, train, and support foster and adoptive parents
Complete home studies, background checks, and recertifications
Conduct home safety visits and provide ongoing support
Match children with appropriate foster and adoptive homes
Work with DHHR, schools, therapists, and community providers
Provide guidance, crisis support, and advocacy for children and families
Participate in on-call rotation
