About the position
The Home Care Registered Nurse position at BrightStar Care of St. Croix Valley involves providing care management services in a home care environment. The role includes overseeing client care, performing assessments, and coordinating with other healthcare services while ensuring compliance with quality and safety guidelines. The position requires a compassionate and patient individual who can build trust with clients and their families, primarily working in the field with minimal time spent in the office.
Responsibilities
- Provides care as defined within the scope of the state's nurse practice act.
- Makes the initial evaluation visit and regularly reevaluates the client's nursing and/or personal assistance needs.
- Assists with the intake of new clients and informs potential clients of our services and differentiator.
- Provides those services requiring substantial and specialized nursing skill.
- Initiates appropriate preventive and rehabilitative nursing procedures and counsels client and family on nursing related needs.
- Prepares clinical and progress notes and summaries of care.
- Assists in the development of organizational goals and agency performance improvement activities.
- Administers agency policies and procedures.
- Assists with assessing the competency of staff to promote quality, continuity and safety of client care.
- Provides information to be used for statistics, reports and records for purposes of evaluation and reporting of organizational activities.
- Assures proper maintenance of clinical records in compliance with local, state and federal laws.
- Promotes home health referrals in the health care community.
Requirements
- Registered nurse with current unencumbered license to practice professional nursing in Minnesota and Wisconsin.
- Minimum of one (1) year of documented experience required.
- Two (2) or more years of clinical experience with one (1) year experience in a home health agency, primary care clinic or health facility preferred.
- Excellent computer skills, 100% digital charting.
- Flexibility and good time management skills are a must.
- Possess exceptional nursing assessment skills.
- Comply with accepted professional standards of practice.
- Excellent interpersonal skills and customer service skills.
- Analytical/Critical thinker and problem solver.
Nice-to-haves
- Experience with, or desire to, participate in marketing and sales.
- Case management skills a plus.
- Understanding of regulatory compliance such as OSHA and HIPAA.
