About The Position

Under the leadership of the department director, the inpatient Heart Failure Program Manager is responsible for the development, implementation, and management of the heart failure program across the continuum of care using a multidisciplinary approach to patient centered care. The role ensures evidence-based, high-quality, and cost-effective care for heart failure patients while improving clinical outcomes, reducing readmissions, and supporting organizational goals related to quality metrics, regulatory compliance, and patient experience.

Requirements

  • 3–5 years of cardiovascular or heart failure clinical experience required
  • Bachelor’s degree required
  • Current California RN license
  • Heart Failure certification (e.g. CHFN) within 6 months of hire
  • Basic Life Support (BLS) required
  • Strong leadership and interdisciplinary collaboration skills
  • Knowledge of heart failure clinical guidelines and disease management
  • Knowledge of quality improvement methodologies (e.g., Lean, Six Sigma)
  • Data analysis and improvement expertise
  • Excellent communication, organizational, and project management skills
  • Ability to drive change and influence clinical practice
  • Strong interpersonal skills
  • Ability to manage direct reports

Nice To Haves

  • Prior program management, quality improvement, or leadership experience preferred
  • Experience with data analysis and performance metrics strongly preferred
  • Team/workgroup participation experience preferred
  • Demonstrated leadership, problem-solving, and critical thinking skills
  • Familiarity with regulatory standards preferred
  • Master’s degree (MSN, MHA, or related) preferred
  • Advanced Cardiac Life Support (ACLS) preferred
