About the position

The Health Home Coordinator (Adult) at Penquis is responsible for providing care coordination and case management to adults with approved mental health diagnoses. This full-time position involves remote work with required travel and in-person meetings in the Dover-Foxcroft and Bangor areas, depending on program needs. The role emphasizes a team-based approach to care, integrating behavioral and physical health needs.

Responsibilities

• Work with individuals to address behavioral health needs, integrating physical health and medical needs.
• Participate in a team-based approach to care by collaborating with supervisors, Peer Support Specialists, Clinicians, Nurse Care Managers, and Psychiatric and Medical Consultants.
• Provide accurate clinical assessments and annual assessments.
• Independently track all Mainecare review dates, assessment reviews, and treatment plan due dates for a year at a time.
• Participate in ensuring the delivery of crisis intervention and resolution services, providing follow-up services to ensure that a crisis is resolved and assists in the development and implementation of crisis plans.

Requirements

• High School Diploma and full or conditional MHRT-C certification required.
• 2-3 years' experience working with children, adolescents, or adults with disabilities.
• Must have a valid State of Maine driver's license in good standing and reliable insured transportation.
• Available to work flexible hours to meet the needs of clients and families.
• Must comply with Penquis background check requirements with satisfactory results.
