As part of the Community Health Planning, Promotion, and Communication Team, the Health Education Program Consultant will improve community health by participating in the planning, implementation, and evaluation of programs and activities designed to encourage healthy behaviors through individual, policy, system, and environmental change. This position is responsible for conducting the full range of activities required to prepare, submit, and manage grant proposals. This includes researching, compiling, writing, and editing grant applications, as well as developing templates to streamline the grant proposal process. This position requires the individual to be self-motivated and work independently with minimal supervision. Hours will be flexible to accommodate after-hours/weekends needs of the community. Objectives will be met by: Understanding chronic disease risk factors and barriers to health in our community. Employing evidence-based practices to reduce or prevent the risk factors that contribute to chronic disease. Using community-oriented, participatory models for creating successful community health promotion initiatives. Planning and coordinating the use of educational resources to motivate and assist individuals and groups to adopt and maintain healthful practices and lifestyles.
