About the position

The Health Coach at CVS Health is responsible for telephonically reaching out to the Medicare population to enhance medication adherence and improve health outcomes related to diabetes and blood pressure control. This role involves assessing members' health statuses, providing education, and addressing barriers to adherence by utilizing the member's benefits. The Health Coach employs a collaborative process of assessment, planning, implementation, and coordination to engage and empower members in their health decisions.

Responsibilities

  • Conduct assessments of members' health status using motivational interviewing and healthcare knowledge.
  • Utilize influencing and motivational interviewing skills to ensure member engagement and promote behavior changes.
  • Partner with members to identify opportunities for integration into available internal/external programs.
  • Consult with management to overcome barriers in assisting members to meet their health goals.
  • Provide coaching, information, and support to empower members to make independent healthcare choices.
  • Interpret and utilize guidelines to positively impact members' health.
  • Facilitate members' understanding of their health status by providing up-to-date healthcare information.
  • Encourage members to actively participate in healthcare decisions with their providers.
  • Develop proactive strategies to address issues and enhance health outcomes.
  • Integrate opportunities to enhance members' overall wellness.

Requirements

  • 1-3 years of experience with Microsoft Office.
  • 3-5 years of practice experience in Renal, Diabetes, Hypertension, Primary Care/Geriatric, or Oncology.
  • Experience in Managed Care and Medicare.

Nice-to-haves

  • Healthcare licensure and/or certifications (RN, LPN, MSW, LCSW, etc.) highly preferred.
  • 5 years of practice experience in Renal, Diabetes, Hypertension, Primary Care/Geriatric, or Oncology.
  • Previous telephonic coaching, education, or case management experience.
  • Excellent computer skills including MS Office applications.
  • Ability to work independently and manage time effectively.
  • Strong analytical and problem-solving skills.
  • Effective communication, organizational, and interpersonal skills; bilingual is a plus.
  • Experience in facilitating positive behavioral change in clients.
  • Background in motivational interviewing and active listening skills.
