About The Position

Employee is a trained home visitor (Para-Professional) for the HANDS program in the state of Kentucky. Credentials will be kept on file and updated on a yearly basis or as changes arise. The workstation will be Rowan County Health Department with travel outside Rowan County determined by District caseload needs. Travel will be reimbursed at the approved state rate.

Requirements

  • Credentials will be kept on file and updated on a yearly basis or as changes arise.
  • Workstation will be Rowan County Health Department with travel outside Rowan County determined by District caseload needs.
  • Travel will be reimbursed at the approved state rate.
  • Completes and submits required HANDS documentation and billing per required policy specifications and timelines.
  • Participates in Ongoing Home Visitor Supervision per required HANDS Policy.
  • Provides weekly completed number of visits from previous week and weekly schedules to supervisor.
  • Maintains confidentiality of program information obtained from home visit(s) and exchanged while communicating with community and with support providers.
  • Completes yearly review and signature of the HANDS Code of Ethics and Home Visitor Safety materials per policy requirements.
  • Maintains strength-based communication within the workplace, with community partners, with HANDS team and Program Management and with HANDS families.
  • Confers with Program Coordinator and/or Supervisor in cases involving potential collaboration with other service providers.
  • Completes and documents required training on an Individual HANDS Training Log including Tier II Premium.
  • Maintains required Productivity expectations, per HANDS policy, based on the weighted caseload, not number of individual families.
  • Demonstrate fidelity regarding the delivery of Growing Great Kids materials.
  • Attends team meetings and in-service training as required by program protocol.
  • Performs other duties, as required that are reasonable and applicable to this position as assigned by the Supervisor.

Responsibilities

  • Provides ongoing HANDS services to first time and multigravida families to support identified HANDS Goals.
  • Provides regular planned visits to establish a trusting relationship for voluntary eligible HANDS participant(s) to assist parent(s) and/or families with child development, parenting skills, health services and other needed resources identified as assessment or during visits.
  • Provides ongoing Resource Referral and facilitation of access to other services provided by Community partners/health department that are outside the scope of services approved by HANDS (family planning, mental health services, intimate partner violence services, daycare, prenatal/pediatric health services, Head Start, Substance abuse services, education and employment, and Family Preservation.
  • Completes and submits required HANDS documentation and billing per required policy specifications and timelines. (Home Visit Logs, Supplemental Billing, Forms/Tools (EDS, DV, ASQ, CPCL, etc.), as well as Corrections)
  • Participates in Ongoing Home Visitor Supervision per required HANDS Policy.
  • Provides weekly completed number of visits from previous week and weekly schedules to supervisor.
  • Maintains confidentially of program information obtained from home visit(s) and exchanged while communicating with community and with support providers.
  • Completes yearly review and signature of the HANDS Code of Ethics and Home Visitor Safety materials per policy requirements.
  • Maintains strength-based communication within the workplace, with community partners, with HANDS team and Program Management and with HANDS families.
  • Confers with Program Coordinator and/or Supervisor in cases involving potential collaboration with other service providers. (Ex. Child Protection, legal, intimate partner violence)
  • Completes and documents required training on an Individual HANDS Training Log including Tier II Premium.
  • Maintains required Productivity expectations, per HANDS policy, based on the weighted caseload, not number of individual families. A full weighted caseload requires 60 visits per month for a full-time home visitor.
  • Productivity: Employee must meet or exceed HANDS policy standards regarding productivity for the position description noted above.
  • Demonstrate fidelity regarding the delivery of Growing Great Kids materials.
  • Attends team meetings and in-service training as required by program protocol.
  • Performs other duties, as required that are reasonable and applicable to this position as assigned by the Supervisor. (Ex. Sub-visits)
  • Refrains from use of the GGK curriculum materials outside their licensed agency.
  • Will leave curriculum materials with the licensed organization should either party end this employment agreement.

  • Cell phone reimbursement of up to $75 per month
