Habilitation Counselor

About the position

As a Habilitation Counselor at Devereux, you will work with other dedicated professionals who share your passion for helping individuals in need. You will play a crucial role in helping individuals maximize their independence and reach their human potential. This includes actively participating in the development and implementation of individualized goals and treatment plans, completing progress assessments, and attending clinical meetings to report behavior data. You will also confer with individuals to discuss their options and goals, facilitate communication with families and guardians, and ensure a safe environment for the individuals you support.

Responsibilities

• Help individuals complete daily activities and achieve long-term goals.
• Participate in the development and implementation of individualized goals/treatment plans.
• Complete progress assessments to track improvements and identify additional areas of need.
• Attend clinical meetings and report behavior data for the designated program.
• Confer with individuals to discuss their options and goals.
• Facilitate monthly communication with families/guardians to discuss individuals and complete required documentation.
• Supervise and/or assist individuals in the implementation of program plans.
• Provide for the individuals' health and physical needs.
• Ensure a safe environment for individuals.

Requirements

• Bachelor's degree in Rehabilitation Counselor or related degree.
• Previous experience preferred (not required) working with individuals with psychological, emotional, and/or behavioral disorders.
• Ability to manage multiple tasks.
• Proficient communication skills.
• Proficient computer skills (Outlook, SharePoint, EHR, Microsoft Office).
• Valid driver's license must be maintained.

Nice-to-haves

• Experience working with individuals with complex post-traumatic stress disorder, co-occurring mental health and substance abuse disorders, and/or autism spectrum disorders.
