About the position

The Floating Case Planner at HeartShare Human Services of New York plays a crucial role in supporting case planners with social services tasks aimed at improving the lives of children in foster care. This position involves creating service plans, monitoring progress, and facilitating family reunification efforts, while also stepping in to cover vacant case planner roles as needed.

Responsibilities

  • Creates service plans in partnership with children, biological families, and foster families to address various needs.
  • Monitors safety, well-being, and progress by facilitating regular casework contacts and documenting in progress notes.
  • Schedules and supervises mandated visits between children and their natural families, conducting necessary field visits.
  • Attends weekly supervision meetings to discuss case situations and updates with the supervisor.
  • Attends Family Court hearings on assigned cases, providing testimony and following up on court directives.
  • Participates in Family Team Conferences and other meetings to promote permanency for children and families.
  • Works closely with Integrated Services to monitor medical and mental health needs of children and families.
  • Utilizes data collected to measure and monitor performance and ensure outcomes are met.
  • Serves as alternate support coverage by facilitating Family Team Conferences.
  • Performs other tasks as assigned within the reasonable scope of duties.

Requirements

  • Bachelor's degree or equivalent experience.
  • Knowledge of the Child Welfare System is preferred.
  • Excellent written, oral, organizational, and interpersonal communication skills.
  • Proficiency in Microsoft Office, including Outlook and Excel.
  • Computer proficiency, including knowledge of Connections.
  • Ability to use public transportation to travel throughout New York City or use an Agency vehicle when available.

  • Experience in social services or case management.
  • Familiarity with family dynamics and child development.
