About The Position

This position involves conducting fitness consultations, providing exercise education, and developing clinical exercise prescriptions for patients, employees, and community members. The role requires monitoring patient progress, collaborating with healthcare providers, and promoting health and wellness through various communication channels. The specialist will also plan and implement clinical and community programs, maintain records in compliance with HIPAA, and contribute to quality monitoring reports. A key aspect of the role is utilizing the latest fitness research and medical knowledge to evaluate and provide health and fitness services, and to research and implement evidence-based programs.

Requirements

  • Applicant must have a valid, unrestricted insurable driver’s license.
  • Resumes and references are required.
  • Bachelor of Science in Exercise Science, Kinesiology and/or related field or equivalent experience.
  • Completion, or requirement to obtain, within six (6) months of starting employment the following certifications: Basic Life Support (BLS) and Automated External Defibrillator (AED), American College of Sports Medicine (ACSM) or other NCCA accredited Personal Training Certification.
  • Must be an enrolled member of the Navajo Nation or other federally-recognized Indian tribes for preference.

Responsibilities

  • Conducts fitness consultations and/or information presentation sessions and orients patients who fall within two (2) specific categories; low risk (those undiagnosed or free from chronic disease) and moderate-high risk (those who display multiple risk factors or are diagnosed with chronic disease such as diabetes).
  • Independently monitors and evaluates the progress of patients, with safety as the number one priority.
  • Provides up-to-date exercise education to patients, employees and community members.
  • Retrieves and reviews patient's medical, laboratory, and pharmacy history using the Electronic Health Record prior to conducting the initial fitness assessments.
  • Conducts clinical exercise prescription, fitness assessments, and patient consultation and collaborates with healthcare providers in accordance with proper exercise guidelines.
  • Able to immediately recognize signs and symptoms of exhaustion, changes in behavior and cognitive ability during physical activity.
  • Plans and facilitates group activities and training for clients, staff, and community members in order to expand services and increase public awareness of chronic disease control and prevention.
  • Promotes health and wellness activities via telephone, social-media, and internet in addition to other communication options.
  • Communicates effectively with medical health providers regarding patient's changes in blood glucose levels, subjective limitations or concerns due to exercise and / or appropriate referrals.
  • Provides consultation to professional and paraprofessional staff and to other programs at the local, tribal, state, and federal levels regarding the effects of exercise on chronic disease.
  • Participates in routine records maintenance in compliance with the Privacy Act, HIPAA and FDIHB policies.
  • Completes all departmental Quality Monitoring Reports and forms in order to properly track patient, program and employee progress.
  • Plans, designs, and implements clinical programs that target special population groups.
  • Utilizes latest fitness research and medical knowledge in providing and evaluating health and fitness services.
  • Researches evidenced based programs to implement for patients and community members after receiving approval from direct supervisor.
  • Plans, designs, and implements fitness programs and events; establishes community partnership and collaboration for resources, data collection and evaluation methods.
  • Provides a final report for all events, programs, and services provided by FDIHB Wellness Department within a timely manner.
  • Ensures effective communication between and among the multi-disciplinary teams to ensure timely and effective completion of event projects or campaigns.
  • Ability to work weekends and holidays.
  • Consults with other members of FDIHB departments regarding clinical application and services.
  • Performs other duties as assigned.
