About the position

The family practice nurse practitioner/physician assistant is responsible for providing continuing healthcare service to individuals and families, irrespective of disease, age, gender, or body system. The FNP is responsible for delivery of quality medical care to patients of the FHN and functions as a member of the multi-disciplinary health care team within the scope of his/her practice. His/her job description entails the delivering of a variety of healthcare services, including preventive, chronic, and acute services. He/she performs diagnoses and treatment of illnesses, as well as carries out health-risk assessments, routine checkups, screening tests and immunization, and counseling to individuals on ways to maintain a healthy lifestyle, so as to prevent diseases. The role of this nurse practitioner in a healthcare facility also includes managing chronic illnesses and sometimes working with specialty physicians in coordinating care to patients.

Responsibilities

  • Maintains clinical expertise and competency as well as demonstrates knowledge of simple procedure to deliver quality healthcare.
  • Provides direct patient care at clinic site, including extended hours in clinics as scheduled.
  • Examines, diagnoses, treats and/or refers patients on schedule/provider panel.
  • Prescribes pharmaceuticals and treatments regimens as appropriate to assessed medical conditions.
  • Refer clients for appropriate specialty care services, lab, x-ray, screening exams for patient’s management and care.
  • Reviews all lab, x-ray and diagnostic tests for patient under his/her care in a timely manner and makes appropriate decisions following the standards of care.
  • Provides continuity of care to provider panel and all assigned patients.
  • Maintains complete and accurate medical records with completion of charting as outlined by the health Center.
  • Ensures that patient’s and family members are educated about diagnosis and the documented plan of care.
  • Acts as a resource in facilitating communication between all members of the health care team.
  • Participates in specific health promotions, education programs and meetings/trainings/staff meetings/huddles and other clinic functions as required by the center.
  • Maintains professional courtesy at all times.
  • Actively participates in the CPI program and Risk Management.
  • Serves as a resource and positive, proactive member of the Medical Staff Team.
  • Identifies, evaluates and prioritizes patient and family needs and calls upon his/her supervising physician to meet all needs of the patient.
  • Performs all related duties as assigned.
  • Be proactive in the goals set by PCMH (Patient Centered Medical Home).
  • Maintain the orientation/reorientation as set by Frontera Healthstream.
  • Attend all mandatory meetings.
  • Is familiar with PCMH standards and works as part of the medical team to provide patient centered care.

Requirements

  • Professional highly trained individual that ensure quality and comprehensive care to both children and adults on a continual basis, with strong emphasis on public health education and prevention of illnesses.
  • Maintains a good knowledge of community living to understand the specific needs of people from various cultural backgrounds.
  • Maintains knowledge of Center’s protocols, medical terminology and appropriate abbreviations.
  • Ability to provide timely and clear documentation.
  • Ability to operate office equipment such as computers and phone.
  • Must hold Texas Advanced Nurse License, Hold a Master’s degree.
  • Maintains continuing education credits as required by Texas Board of Nurse Examiners.
  • Current Cardiopulmonary Resuscitation (CPR) for Healthcare Professionals Certification.
  • Must have current DEA and other such certificates to legally operate as a practitioner in the state of Texas.

Nice-to-haves

  • Spanish speaking and writing skills.
  • One (1) year of practice experience as a Nurse Practitioner giving direct patient care.
