About the position

The Family Networks Service Coordinator is a full-time role focused on coordinating and managing services for families within the community. This position requires a Bachelor's degree in Human Services or a related field and involves a hybrid work model, with three days in the office and two days remote. The coordinator will conduct family conferences, case management, and collaborate with various service providers to ensure family success.

Responsibilities

  • Coordinate and manage services between families and providers.
  • Conduct family conferences and case management.
  • Collaborate effectively with service providers, DCF, and community supports.
  • Attend case conferences, clinical meetings, and educational meetings for families as needed.

Requirements

  • Bachelor's degree in Human Services or a related field.
  • Two years of clinical and/or direct care experience in Human Services.
  • Experience as a Service Coordinator working with families involved with DCF is a plus.
  • Valid driver's license.
