The Family Model Provider (FMP) role at AdvanceCare involves providing non-medical, in-home care to individuals with intellectual and developmental disabilities. Responsibilities

• Provide companionship and conversation to clients.
• Assist clients with mobility and physical activities.
• Help with meal preparation and planning.
• Remind clients about medication and appointments.
• Assist clients with grooming and bathing needs.
• Support clients with toileting and incontinence issues.
• Perform grocery shopping and run errands for clients.
• Provide transportation services as needed.
• Ensure safety supervision for clients.

Requirements

• Must have a valid Driver's License.
• Ability to pass a background check.
• Experience in caregiving (1 year preferred).
• CPR/First Aid Certification (preferred).
• Medication Administration Certificate (preferred).
