About the position

The Emergency Housing Care Coordinator position is a full-time role focused on providing emergency and transitional housing options through a Housing First model. This role emphasizes person-centered, holistic care to assist clients in achieving housing stability and independence. The coordinator serves as the primary contact for clients, facilitating case management and coordinating necessary resources and support.

Responsibilities

  • Conduct intake, assessment, and orientation for clients entering the Emergency Housing program.
  • Ensure timely documentation of chart records, assessments, care plans, and case notes.
  • Utilize SCOUT/Service Point/CaseWorthy databases for service recording.
  • Assist clients in obtaining resources, benefits, and entitlements.
  • Conduct inspections of emergency and permanent housing sites as required.
  • Coordinate a support team for each client, including Ryan White Case Manager and other supports.
  • Assist clients in securing permanent housing in the community.
  • Engage clients using housing first principles with a trauma-informed approach.
  • Build relationships with landlords, homeless service providers, and health organizations.
  • Complete other duties as assigned.

Requirements

  • Bachelor's degree in Social Work or a related field preferred.
  • Broad understanding of HIV/AIDS, mental health disorders, substance use disorders, and homelessness.
  • Knowledge of de-escalation techniques, motivational interviewing, housing first principles, trauma-informed care, and harm reduction.
  • Experience with shelter and housing services, mental health services, and substance use disorder services.
  • Understanding and experience with client tracking software.
  • Ability to work independently and as part of a team.
  • Excellent writing and communication skills.
  • Proficient in Microsoft Office software, especially Word, Excel, and Outlook.
  • Reliable transportation and willingness to work with clients at multiple housing sites.

Nice-to-haves

  • Experience in crisis intervention and working with individuals in crisis.
  • Familiarity with community social, health, and welfare agencies.
