Responsibilities

• Market home health services to residents.
• Direct resident care, conduct resident assessments and develop plan of care, collaborate with physicians and other health care professionals in implementing and maintaining plan of care, assist with medication administration and treatments.
• Monitor resident for changes in condition, medication & nutritional needs and update care plan, physician, and family as necessary.
• Ensure nursing services are provided according to the resident's plan of care with home health access available 24 hours per day, 7 days a week.
• Develop and maintain an on-call coverage system for nursing and resident care services that provides coverage on evenings, nights, weekends, and holidays.
• Participate in the home health advisory committee and development of administrative and resident care policies and procedures relating to the agency.

Requirements

• Licensed Practical or Registered Nurse, as required by the state.
• Three years of experience as a nurse with two years administrative/supervisory experience in home health or community health.
• Completion of a bachelor's degree in nursing or related field, highly preferred.
• Completion of a master's degree program in nursing, including public health and/or administrative studies preferred.
• Public Health Nursing certification, if required.
