About the position

The Residential Direct Care position involves providing direct care and support to individuals in a residential setting. The role focuses on promoting the quality of life for residents by assisting them in various aspects of daily living, personal growth, and community involvement. The position requires effective communication, adherence to policies, and collaboration with team members to ensure the well-being of individuals served.

Responsibilities

  • Provide direct care coverage and support for individuals served during assigned shifts.
  • Work with individuals to promote growth in quality of life areas such as Individual Control, Respect and Dignity, Relationships, Community Membership, Personal Growth & Accomplishments, and Personal well-being.
  • Assist individuals in working on areas identified in their Individual Support Plans.
  • Support individuals in problem solving, personal care, medical care, socialization, leisure activities, interpersonal skills, household management, money management, and activities of daily living.
  • Assist in the development and implementation of behavior support plans.
  • Know emergency procedures and basic emergency facts regarding all individuals at the location.
  • Complete necessary documentation including Communication Log, Case Log, Daily Progress notes, Incident Reports, and Medication Charts.
  • Transport and/or accompany individuals to appointments and community-based activities as needed.
  • Maintain Triangle property in good and sanitary condition.
  • Attend and participate in supervision and staff meetings as scheduled.
  • Attend training as directed by the Program Manager.
  • Accomplish all tasks as assigned.

Requirements

  • High school diploma or GED.
  • One to three months related experience and/or training, or equivalent combination of education and experience.
  • Ability to verbally communicate, read, analyze, interpret, and write documents.
  • Ability to respond to inquiries or complaints from regulatory agencies or family members.
