About the position

The Digital Health Navigator Specialist (DHNS) plays a crucial role in the THRIVE Digital Equity Program at Boston Medical Center, focusing on improving access to digital health tools and bridging the digital divide for patients. This position involves assisting patients with telehealth access issues, providing digital health literacy training, and ensuring equal access to BMC's digital tools, including the patient portal and telehealth platform. The DHNS is responsible for assessing patients' digital health needs, addressing barriers to access, and collaborating with community partners to enhance digital health support services.

Responsibilities

  • Develop and enhance a weekly list of patients needing digital health support.
  • Proactively reach out to patients before telehealth appointments to assess their needs.
  • Assist patients without internet access by connecting them with service partners.
  • Provide guidance for MyChart registration and navigation, both over the phone and in-person.
  • Conduct follow-up calls to confirm resolution of needs and offer additional assistance.
  • Collaborate with community partners to identify organizations that can assist patients.
  • Document all patient interactions and ensure compliance with departmental guidelines.
  • Collaborate with the THRIVE Digital Equity program to improve access to telehealth platforms.
  • Train Digital Health Navigators and provide guidance to medical students and volunteers.
  • Participate in grant writing and preparation of program summaries.

Requirements

  • Associate's Degree required; Bachelor's Degree preferred.
  • Minimum of 2 years of experience in a healthcare setting, preferably in community health or digital health support.
  • Strong knowledge of digital health tools and resources, including telehealth platforms and patient portals.
  • Empathy and cultural competence in supporting diverse patient populations.
  • Ability to build effective working relationships with patients and community partners.
  • Proficiency in technology and computer systems, including electronic medical records and telehealth systems.
  • Strong organizational, critical thinking, and problem-solving skills.
  • Excellent verbal, written, and interpersonal communication skills.

Nice-to-haves

  • Bilingual or multilingual skills (beyond English) are preferred.
