About the position

The Diabetes Education Care Coordinator - RN at the Department of Veterans Affairs is responsible for delivering knowledge-based care to clients with diabetes. This role involves assessing, planning, implementing, and evaluating educational needs to facilitate patient development and skill training. The coordinator will utilize innovative teaching strategies and assessment skills to enhance patient education, self-management, and overall satisfaction throughout the healthcare continuum.

Responsibilities

  • Provide patient management and care coordination in outpatient settings.
  • Assist Veterans, family members, and caregivers in accessing appropriate options and services for complex healthcare needs.
  • Understand and apply instructional design related to education and training.
  • Demonstrate knowledge of professional standards related to patient diabetes care.
  • Perform learning needs assessments and develop educational trainings for patients.
  • Develop entry-level coaching and mentoring skills.
  • Evaluate current practices and formulate appropriate diabetic outcomes for groups of patients.
  • Act as a consultant in patient diabetes care principles and performance measures.
  • Review current nursing literature for new diabetic trends and propose practice revisions based on evidence-based research.
  • Utilize best practices and benchmarking to establish realistic goals for patient diabetes care initiatives.
  • Provide consultation and collaboration through participation in organizational committees.

Requirements

  • U.S. Citizenship.
  • Graduation from a school of professional nursing approved by the appropriate accrediting agency.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a state, territory, or commonwealth of the U.S.
  • Proficiency in basic written and spoken English.
  • Ability to pass a pre-employment physical examination.
  • Participation in the seasonal influenza vaccination program.
