About The Position

This position involves providing professional case management for adults with developmental disabilities, focusing on stability, safety, and long-term support. The role includes coordinating various services, advocating for individuals, assessing and monitoring client needs, providing crisis-related support, and handling program operations and administrative responsibilities. The Crisis program operates 24 hours a day, but the Crisis Case Manager hours are generally Monday through Friday, 8a-5p, with weekends and holidays off. This position requires travel throughout Cumberland, York, and Oxford Counties and allows for partial telework with management approval. The Department of Health and Human Services (DHHS) is dedicated to promoting health, safety, resilience, and opportunity for Maine people, providing essential health and social services. The Office of Aging and Disability Services (OADS) within DHHS supports adults with developmental disabilities and their families during and after crisis events.

Requirements

  • A bachelor’s degree from an accredited educational institution in Social Work/Social Welfare; OR a bachelor’s degree in a related social service/social welfare/social work area which includes at least 12 courses in behavioral science, social science, or social work.
  • At least 1 year of experience working providing support services and/or direct care to clients and families with developmental disabilities, or directly related human service field.
  • Must have or be eligible for conditional or full licensure at the Licensed Social Worker (LSW) level as issued by the Maine State Board of Social Worker Licensure.
  • Possession of a valid Class C Maine Motor Vehicle Operator's License.

Responsibilities

  • Provide professional case management for adults with developmental disabilities, focusing on stability, safety, and long-term support.
  • Coordinate housing, employment, medical, psychiatric, and community supports.
  • Review and document person-centered plans, care plans, and behavior plans.
  • Advocate for individuals and ensure services align with their needs and goals.
  • Visit clients regularly to assess safety, living arrangements, and support systems.
  • Maintain accurate case records, summaries, reports, and correspondence.
  • Evaluate service effectiveness and recommend continuation, modification, or termination.
  • Participate in crisis resolution using established intervention techniques.
  • Collaborate with Adult Protective Services and Crisis Prevention/Intervention Services.
  • Support district crisis teams as needed and assist with emergency transitions to waiver programs.
  • Interpret and apply policies, regulations, and program procedures.
  • Prepare and review correspondence, financial statements, and operational documents.
  • Assist with public education, complaint review, investigations, and workflow improvements.
  • Represent the program at meetings and support special projects assigned by the Crisis Program Administrator.
