About the position

Near North Health, a longstanding pillar of Chicago's healthcare landscape, is seeking a visionary and compassionate Healthy Families Coordinated Intake to join our dedicated team. Since our inception in 1966, we have remained committed to our mission of providing accessible, affordable, and high-quality primary healthcare services to all, regardless of their financial circumstances. As Healthy Families Coordinated Intake you will play a pivotal role in our mission to serve primarily low-income uninsured or underinsured residents of Chicago's medically underserved communities. Your attention to detail in screening families to identify their strengths and needs, will further solidify our position as one of the largest providers of community-based primary care in the city.

Responsibilities

  • Conduct Intake: Home Visitation Programs using a universal tool to determine which home visit program that participants are eligible and has availability for the participant.
  • Support Referrals: Monitor all program capacity levels and send referral to best-fit programs as needed to ensure caseloads do not drop below.
  • Collaboration: Know, engage, and collaborate with community partners and ancillary organizations related to enrolling eligible families in home visiting.
  • Engage: Learn, engage and collaborate with key individuals, organizations, and ancillary agencies that support maternal, child and family health in the community we serve.
  • Advocate: Advocates for safe and healthy communities. As directed, performs outreach activities to community.

Requirements

  • BS/BA in Human Services field.
  • Two or more years of experience in Social/Human Services field.
  • Program experience and some quality improvement and quality assurance experience desirable.
  • Demonstrated knowledge of community resources and social services providers.
  • Excellent verbal and written communication skills.
  • Computer operations and data entry skills.
