About the position

The Community Resource Navigator at Cherry Street Mission Ministries is responsible for promoting health and wellness among individuals facing extensive physical and mental health challenges. This role involves connecting individuals with community resources, managing case loads, and providing follow-up support to reduce barriers to care. The Navigator also plays a critical role in infectious disease response, including COVID-19 coordination, and is expected to conduct crisis interventions and risk assessments while upholding the organization's mission and values.

Responsibilities

  • Connect individuals with appropriate community resources and provide follow-up support to reduce barriers.
  • Identify effective placement opportunities for high acuity individuals outside the scope of Cherry Street services.
  • Coordinate transportation and oversight of Isolation/Quarantine Site for guests testing positive for COVID or other infectious diseases.
  • Manage case load effectively, completing documentation and meeting requirements outlined in Standard Operating Procedures.
  • Conduct crisis intervention and debriefing, including verbal de-escalation.
  • Apply understanding of stages of change and motivational interviewing techniques.
  • Perform risk assessments and implement harm reduction techniques during home visits.
  • Attend internal and external training opportunities to advance skill set and attend mandatory staff meetings and trainings.
  • Responsible for incident reporting and responding to emergencies accordingly.
  • Support the organization by upholding its mission, vision, and values.

Requirements

  • High School Diploma or equivalent required.
  • Preferred: Associate's Degree in Social Work, Counseling, or related field.
  • At least 2 years of experience in the mental health field, shelter care, or similar.

Nice-to-haves

  • Mental Health First Aid Certified or willing to complete within the first year of employment.
  • CPR Certified or willing to complete within the first year of employment.
  • Suicide Prevention Training or willing to complete within the first year of employment.
  • Trauma-Informed Care Training or willing to complete within the first year of employment.
