About The Position

The Community Outreach Specialist is a full-time position working 37.5 hours per week, with hours that may include nights and weekends as needed. This role is part of the Womens Health Partnership and is responsible for coordinating and participating in the development, implementation, and evaluation of diversity and health equity initiatives across the organization. The specialist also serves as a liaison to system-wide departments requiring educational tools and coordinates and participates in performance improvement projects.

Requirements

Bachelor's Degree required, or equivalent combination of education and related experience.
1-3 years of relevant experience, required.

Responsibilities

Serves as a liaison to system-wide departments requiring educational tools; collaborates with management to create a vision and plan for health literacy activities throughout the organization.
Administers and analyzes readability pre and post tests and makes recommendations based on findings.
Provides stakeholders with all necessary documentation and information; explores additional programs and training.
Monitors inventory of patient education materials.
Provides periodic updates, reports, and makes recommendations to management regarding Health Literacy services in meeting the needs of patients, healthcare providers, and the community.
Provides resources and educates staff on health literacy in an effort to deliver services that align with internal and government regulations.
Conducts meetings, disseminates information, collects data, writes reports, and seeks expansion of diversity and health equity initiatives across the organization.
Coordinates and participates in performance improvement projects, including writing grants, coordinating research projects, and developing Center for Equity of Care documents.
Operates under general guidance and work assignments are varied and require interpretation and independent decisions on course of action.
Performs related duties as required.
