About The Position

Community Medical Services (CMS) is hiring a Community Impact Specialist to assist patients recovering from addictions to opioids and other illicit substances. This role involves fostering relationships with community partners such as coalitions, health homes, sister agencies, specialty providers, local hospitals, clinics, law enforcement, and fire and EMT services. The specialist will serve as an integral point of contact for care coordination between clients and community agencies, supporting CMS clinic staff in providing a safe environment for individuals starting their recovery journey. The position is part of a mission to help individuals recover from substance use disorders in a supportive, engaging, and fulfilling work environment.

Requirements

Peer Recovery Support Cert Required
Ability to travel within the region, including to all CMS locations as needed
Culturally competent and sensitive to individualized client treatment needs
Valid Driver License and Motor Vehicle Clearance (satisfactory driving record for 39 months)

Nice To Haves

Experience in substance abuse rehabilitation and/or opioid treatment programs (preferred)
Bilingual (preferred)

Responsibilities

Foster relationships with community partners, including community coalitions, health homes, sister agencies, specialty providers, local hospitals, clinics, law enforcement, and fire and EMT services.
Serve as an integral point of contact for the coordination of care between opioid treatment program clients and community agencies.
Support CMS clinic staff in providing a safe place for anyone looking to start their recovery journey.
Identify and access community resources.
Provide timely and accurate documentation, including for all client/agency encounters, coordination of care efforts, and community outreach activities, presentations, and meetings.
Maintain outreach program data in multiple databases, including QuickBase, Methasoft, and CRM.
Maintain appropriate coordination of care with all systems and provide documentation as requested from system partners.
Provide support, including intake documentation, to special populations and projects.
Resolve client altercations, manage crisis episodes, and assist in conflict resolution.
Attend provider and community meetings for the purpose of advocating, educating, and supporting system partner relationships.
Work with the project management team to support program development and implementation.
Receive and process referrals for community support from multiple CMS clinics and community system partners.
Complete documentation within 24 hours of shift.
