About the position

The Seacoast Public Health Network (SPHN), a program of Lamprey Health Care, is seeking a Community Health Worker (CHW) who promotes resources designed to increase access to quality health care in the 23 towns that comprise our public health region. This position is both remote and community-based, requiring commuting distance to Raymond, NH and the surrounding towns. The CHW will be responsible for connecting community members to culturally and linguistically competent COVID-19 testing and vaccination sites, cultural mediation among individuals, communities, and health and social service systems, care coordination and system navigation, coaching and social support, and advocating for individuals and communities. The role also involves identifying and mitigating barriers in health care access, educating healthcare providers about community health needs, advocating for vulnerable populations, building community capacity to address health issues, and supporting other areas of public health work including substance misuse prevention and emergency preparedness.

Responsibilities

  • Connecting community members to culturally and linguistically competent COVID-19 testing and vaccination sites
  • Cultural mediation among individuals, communities, and health and social service systems
  • Care coordination and system navigation
  • Coaching and social support by advocating for individuals and communities
  • Identifying and helping to mitigate barriers in health care access such as transportation, language, and childcare
  • Educating healthcare providers and stakeholders about community health needs
  • Advocating for vulnerable populations or communities to receive services and resources to address health needs
  • Building community capacity to address health issues
  • Supporting other areas of PHN work including substance misuse prevention and emergency preparedness

Requirements

  • Graduating from Community Health Worker training (enrollment in a training program is available)
  • Ability to multitask, prioritize and operate successfully in a fast-paced environment
  • Strong communication skills (both verbal and written) and attention to detail
  • Excellent technology skills including use of Word, Excel, & PowerPoint, as well as knowledge of social media
  • Well-developed diplomacy skills and respect for diverse perspectives
