About the position

The UNM Medical Group, Inc. is seeking a Full-time Community Health Worker to work in our Truman Health Services Team. This candidate must be an excellent communicator with a desire to be part of a great team! This opportunity is a full-time day shift opening located in Albuquerque, New Mexico. Under general supervision, the Community Health Worker provides services relating to health education, health promotion, and disease identification and prevention. The role involves conducting prevention intervention programs and sharing responsibility for maintaining, enhancing, and presenting health information and resources related to HIV/HCV and other relevant health issues. Additionally, the position performs a range of administrative functions required to ensure efficient program operation.

Responsibilities

  • Assists clients in their homes, community, or clinic setting.
  • Communicates to clients/patients the purposes of the program and the impact it may have on their wellbeing.
  • Helps patients identify socio-economic issues that affect their overall health and develop health/social management plans and goals.
  • Assists in identifying individuals who would benefit from retention in care strategies.
  • Coaches patients in effective management of their chronic health conditions and self-care.
  • Assists patients in understanding care plans and instructions.
  • Motivates patients/clients to be active and engaged participants in their health and overall wellbeing.
  • Assists clients with overcoming barriers to obtaining needed medical care and/or social services.
  • Provides support and advocacy during initial medical visit or when necessary to assure clients' medical needs and referrals required are being conveyed.
  • Builds and maintains positive working relationships with clients, providers, nurse case managers, agency representatives, supervisors, and office staff from diverse cultural and socio-economic backgrounds.
  • Participates in community events, health fairs, and school events to engage individuals to participate in retention in care clinics.
  • Provides clear, understandable, current, and accurate health-related information to clients; provides information and referrals in a way that demonstrates respect, active listening skills, empathy, and patience.
  • Maintains professional growth and development through completion of required departmental competencies.
  • Performs miscellaneous job-related duties as assigned.

Requirements

  • High School diploma or GED.
  • Must complete New Mexico State CHW Certification within 1 year of employment.
  • Completed degree from an accredited institution that is above the minimum education requirement may be substituted for experience on a year for year basis.
  • Possession of a valid unrestricted NM driver's license.

  • Experienced working with the LGBTQ+ community.
  • Experienced working in a community-based multi-disciplinary setting.
  • Bilingual English/Spanish.
