About The Position

The Community Health and Outreach Coordinator is responsible for coordinating the activities of the Community Health Workers/Promotora team, overseeing outreach activities including, tabling, food distribution and large community events. The ideal candidate is skilled at establishing long-term partnerships and has experience developing and managing community health and outreach programs that will increase impact for those served by CCOLE. Requirements

  • Bachelor’s Degree OR a combination of academic education, professional training or work experience, which demonstrates the ability to perform the duties of the position.
  • Minimum of 3 years of direct and relevant experience or relevant college degree.
  • Community outreach experience, especially with a community health center or community-based organization strongly desired.
  • Experience in a similar role with proven track record of relationship-building in low to mid-income communities highly desired.
  • Must have demonstrated commitment to serving the needs of agricultural workers and underserved populations.
  • Bi-lingual English/Spanish speaker required.

Responsibilities

  • Responsible for the execution of a robust annual calendar of CCOLE community health and outreach activities that address the health and wellbeing of specific communities throughout Napa, Solano, and Yolo Counties.
  • Plays a lead role in the planning, coordination, and oversight of signature community health events and activities across Napa, Solano and Yolo Counties including monthly food distribution, Dia de Muertos celebrations, PRIDE events, community health screenings, and others.
  • Maintains and establishes excellent relationships with community partners, Chambers of Commerce and other key stakeholders; providing regular status updates related to identifying community needs and recommendations for partnership opportunities.
  • Tracks and reports on KPIs for CCOLE community health and outreach initiatives to measure impact and effectiveness.
  • Integrates the community health and Promotores team with mobile health team to deliver services and health education to agriculture workers, students, unhoused residents, senior citizens and other vulnerable populations.
  • Supports Associate Director coordinating the Promotora program and creates engaging culturally relevant activities for vulnerable populations and events that increase awareness of CCOLE services and engages community members in being more actively involved with their personal health.
  • Oversees the recruitment and management of a robust volunteer core that supports CCOLE’s community health events, operations, and other initiatives.
  • Utilizes best practices in volunteer management including the use of technology systems for scheduling and tracking volunteer activity, engagement, and recognition.
  • Ensure community health and outreach activities are within budget and support grant deliverables.
  • Ensure collaborative working relationships between the community health and outreach team members with internal departments including Health Center Operations, Clinical, Dental, Pharmacy, Behavioral Health, Patient Services and others to identify community and patient population needs.
  • Performs other duties as assigned.
