About the position
Rivers Health is looking for a full-time Chronic Care Management/Population Health Certified Medical Assistant (CMA) to coordinate and facilitate the implementation of assessments, care plans, and interventions for a designated patient population. This role focuses on understanding patient health and social situations, including identifying risk factors and social determinants of health.
Responsibilities
- Coordinate and facilitate timely implementation of assessments and care plans for patients.
- Identify risk factors and social determinants of health for the patient population.
- Collaborate with healthcare providers to ensure appropriate interventions are implemented.
- Communicate effectively with patients of all ages, including neonates, pediatrics, adolescents, and geriatrics.
- Prioritize and delegate tasks effectively within the care team.
Requirements
- Graduate of a Registered Certified Medical Assistant Program.
- Must hold an active certification as a Medical Assistant.
- One year of experience in a provider office or hospital setting with direct patient care.
Nice-to-haves
- Experience working with diverse patient populations.
- Knowledge of chronic care management principles.
