The Mobile and Street Medicine program delivers comprehensive, high-quality medical care and social services to individuals experiencing homelessness in San Francisco. Rooted in the mission to "go to the people", the program offers services directly to patients on the street, in encampments, or via a fully equipped mobile medical van. The program operates as an extension of our Integrated Care Center (ICC), expanding the reach of our services beyond the clinic walls. This new and rapidly growing initiative is committed to providing low-barrier, trauma-informed, and person-centered care in the environments where people live. Our multidisciplinary team prioritizes trust, continuity, and access while delivering compassionate, dignified, and high-quality services to some of the city’s most underserved populations. The Clinical Social Worker provides behavioral health therapy and integrated case management to individuals experiencing unsheltered homelessness and complex medical, psychiatric, and social needs. This role combines field-based case management with direct clinical mental health services and plays a key role in strengthening and expanding the Mobile & Street Medicine program’s behavioral health capacity. This position is highly collaborative, flexible, and hands-on. The ideal candidate thrives in dynamic field environments, enjoys wearing many hats, and brings creativity, leadership potential, and a commitment to building new systems of care alongside a multidisciplinary team. This is a full-time, in-person, field-based position. This position may be filled by an ASW practicing under clinical supervision, which will be provided by an LCSW employed by HR360, or by an independently licensed LCSW.
