About The Position

Family Health Centers of San Diego (FHCSD) is seeking a Clinical Site Manager for Physical Rehabilitation Services. This role involves providing operational management, including administrative and clinical oversight, to Physical Rehabilitation Services and activities at assigned clinics. A significant portion of the role (60%) will be dedicated to providing direct patient care as a Physical Rehabilitation Services clinician. The ideal candidate will demonstrate a passion for delivering caring, high-quality care to a diverse patient population, with a special commitment to the uninsured, low-income, and medically underserved. FHCSD has a long-standing mission to provide accessible, high-quality healthcare and supportive services, operating over 90 sites across San Diego County. As one of the largest FQHCs in the country, FHCSD serves over 227,000 patients annually, with a strong focus on those in need. The organization offers a wide range of services, including primary care, behavioral health, dental, vision, and physical rehabilitation.

Requirements

Master’s Degree of Physical Therapy, Chiropractic, or Acupuncture required.
Current California state clinical licensure as Physical Therapist, Chiropractor, or Acupuncturist.
Current American Heart Association healthcare provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training.
Possession of First Aid/CPR certificate issued by an authorized agency.
3 years leadership experience in Healthcare.
2 years experience leading or supervising in Clinical setting required.
Ability to maintain good interpersonal interactions with clients and co-workers as a member of a multi-disciplinary community health team with a diverse multi-cultural population.
Traveling between sites and other locations is required as an essential function of the job.
Must have a car, a valid California driver’s license, and proof of minimum levels of car insurance as required under California law, although limits of $100,000 are recommended.
An acceptable driving record is also required.
California law requires all drivers to obtain a valid California driver’s license within ten days of establishing residency.
Mileage and other reimbursement governed by policy.

Responsibilities

Build positive relationships, inspire others to perform at a higher level, prepare career development plans and retain an accountable workforce.
Coach and counsel staff and administer appropriate disciplinary action in a timely manner.
Provide/ensure staff receive all new hire and annual trainings on time.
Administer day-to-day operations of assigned clinics, and/or assigned programs to include scheduling and patient service management.
Establish operational objectives, and develop and implement plans for meeting tactical and strategic objectives as reviewed by senior management.
Oversee administration of integrated care activities at assigned clinics to include data collection and report generation.
Track site productivity and utilization rates.
Monitor on-going revenue and expenses.
Collect and analyze data, prepare necessary reports, and develop improvement plans as necessary.
Provide direct patient services as appropriate within scope of licensure.
In collaboration Physical Rehabilitation Services Senior Management Team, actively participate in organizational short and long-term planning and implementation activities.
Oversee and directly participate in project planning, implementation, and assessment at assigned clinics.
Complete all required documentation accurately, in a timely and thorough manner, in accordance with department standards.
Ensure documentation complies with patients insurance requirements.
Monitor documentation of direct reports and assigned clinics to ensure maximum revenue realization.
Work effectively as member of a multi-disciplinary treatment team.
Communicate effectively with providers, physicians, case managers, scheduling staff, and department leadership in a timely and appropriate manner.
Review and maintain clinical guidelines.
Assess quality of clinical care.
Participate in clinical peer review.
Advise on standards of care for clinical practice.
Ensure applicable regulatory and department standards are met or exceeded on an ongoing basis (i.e., JCAHO, OSHA, Safety Standards, etc.).
Ensure assigned clinics and/or programs are managed in a fiscally responsible manner to include grants implementation and requirements as directed in collaboration with other FHCSD departments and clinics as applicable.
Ensure assigned site is well maintained and in compliance with FHCSD policies.
Ensure work collaboratively with other managers in addressing and servicing difficult patients.
Ensure Physical Rehabilitation Services staff are kept informed and adhere to policies and procedures that relate to the Physical Rehabilitation Services clinic.
Participate in continuous improvement activities and ensure appropriate Lean tools (i.e., 5S, A3) are deployed and results are evaluated and/or measured.
Perform other duties as assigned.
