About the position

The University of Texas M. D. Anderson Cancer Center is seeking a Clinical Pharmacist to develop and maintain an active clinical pharmacy practice. This role involves participating in inpatient rounds and outpatient clinic coverage, including on-call and weekend coverage as assigned. The Clinical Pharmacist will collaborate with the multidisciplinary healthcare team to optimize pharmacotherapeutic aspects of patient care, manage medication therapy, and provide drug and clinical information to healthcare professionals. The position also includes responsibilities for education and training, clinical support, and adherence to regulatory policies.

Responsibilities

  • Develops and maintains an active clinical pharmacy practice on assigned service(s).
  • Participates in inpatient rounds and outpatient clinic coverage.
  • Collaborates with the multidisciplinary healthcare team to optimize pharmacotherapeutic aspects of patient care.
  • Manages medication therapy of individual patients in collaboration with the healthcare team.
  • Documents activities in the electronic health record appropriately.
  • Provides timely follow-up on clinical effects of medication regimens.
  • Provides drug and clinical information to other healthcare professionals.
  • Develops and communicates specific pharmacotherapeutic plans for individual patients.
  • Provides accurate and timely medication reconciliation.
  • Identifies and intervenes to resolve potential medication problems.
  • Participates in discharge planning for assigned inpatients.
  • Assesses patient's financial status and addresses barriers.
  • Participates in and guides medical staff to comply with institutional standards.
  • Provides effective medication and practice-related education to patients and caregivers.
  • Demonstrates a desire and aptitude for teaching.
  • Serves as an effective preceptor for residents and students.
  • Models professional behavior to mentor and train new clinical staff.
  • Uses technology to facilitate safe and efficient delivery of patient care.
  • Participates in medication-related processes to improve patient care.
  • Adheres to regulatory/compliance policies.
  • Critically analyzes practice for self-improvement.
  • Documents and reviews clinical activity/interventions appropriately.
  • Models service excellence.
  • Attends and participates in committees/meetings.
  • Demonstrates effective oral and written communication skills.

Requirements

  • Pharm.D degree.
  • Texas State Board of Pharmacy Pharmacist license within 90 days of job entry.
  • Pharmacy Practice Residency (PGY 1) and Residency in the area of specialization (PGY 2).
  • Experience as a clinical pharmacist in the area of specialization may substitute for the required residency on a one to one basis.
