Requirements

• Pharmacist credentialed from the Oklahoma State Board of Pharmacy required.
• Bachelor's or advanced degree in Pharmacy required.

Nice To Haves

• Pharmacist licensure by the state of Kansas prior to or within 6 months of hire date preferred
• Board Certified Ambulatory Care Pharmacist credential preferred
• Completion of a PGY-2 Ambulatory Care Pharmacy Residency in lieu of Ambulatory Pharmacist experience

Responsibilities

• Pro-actively manage chronic diseases (e.g., diabetes, hypertension) to prevent complications and improve overall quality of life.
• Perform comprehensive medication management, interpret labs, and safely adjust dosages under collaborative practice agreements.
• Identify financial or insurance barriers and provide targeted education to streamline therapy and ensure access.
• Collaborate directly with providers to streamline workflows and ease provider burnout.
