About The Position

The Clinical Nurse Navigator (CNN) is a professional role model and clinical expert who serves as an advocate and educator responsible for facilitating patients’ and/or caregivers’ experience by guiding patients across the continuum, providing education and support, and helping predict follow-up care needed for treatment and disease process in order to promote medical optimization to ensure best outcomes and best value of care. Registered Nurse responsible for rounding on post-operative cardiac surgery patients and their families, providing comprehensive education on recovery expectations, medication management, vital sign monitoring, and use of a surgeon-shared recovery tracking application. Collaborates closely with cardiac surgeons and the interdisciplinary care team to support patient navigation, coordinate discharge planning, document patient progress, and ensure a seamless transition from hospitalization to recovery. Ideal candidates possess cardiac specialty experience, strong communication and patient education skills, a proactive and self-motivated approach, and the ability to anticipate and address patient care needs with attention to detail.

Requirements

  • One (1) to three (3) years of experience in a specialty area.
  • Graduate of an accredited school of nursing or a diploma program.
  • For new hires after 2012, Bachelor of Science Degree in nursing (BSN) is required within specified timeframe as outlined in the New Hire BSN Agreement policy (126.045).
  • BLS: Basic Life Support/CPR
  • FDL: Valid Driver's License
  • FL RN: FL Registered Nurse License
  • Undergo background screening through The Florida Care Provider Background Screening Clearinghouse.
