About the position

The Clinical Liaison at Select Specialty Hospital plays a crucial role in advocating for patients requiring extended hospital care after critical illness or surgery. This position involves collaborating with healthcare professionals to assess patient needs, coordinating care plans, and facilitating patient transfers to ensure a smooth continuum of care. The Clinical Liaison is expected to embody the values of Select Medical by prioritizing patient care and improving community health outcomes.

Responsibilities

  • Advocate for referred patients in the pre-admission process by collaborating with clinical teams to assess patient needs.
  • Serve as a resource for healthcare professionals by fostering relationships within medical facilities.
  • Coordinate care plans with physicians and families using clinical expertise and problem-solving skills.
  • Facilitate patient transfers and address barriers to accessing specialized post-ICU care.
  • Work with physicians, discharge planners, and families to plan the best continuum of care after an ICU or post-surgical stay.
  • Perform clinical assessments of referred patients and counsel families to assist in care decisions.
  • Partner with hospital case managers and physicians to create smooth pre-admissions and transfer processes.

Requirements

  • Registered Nurse (RN) or Respiratory Therapist (RT) License.
  • Ability to complete clinical assessments of medically complex patients.

  • Demonstrated success in patient advocacy.
  • Superior communication and relationship building skills.
  • Prior healthcare marketing or business development experience.
