About The Position

The Clinical Liaison/Admissions role at Avamere Rehabilitation of Issaquah is a full-time position responsible for acting as a liaison between healthcare providers to ensure continuity of care for residents transitioning from the hospital to home health. This role involves managing referrals, performing qualifying assessments, educating discharge planners, patients, and families about services, and meeting established referral/admissions goals. The position requires strong relationship and sales skills to build long-term relationships that generate resident referrals and admissions. The Clinical Liaison will also deliver education and presentations to hospital staff, participate in community organizations, complete resident evaluations, and maintain regular attendance. Additionally, the role involves serving on committees to enhance hospital relations, attending sales meetings, reporting incidents of fraud, waste, or abuse, participating in compliance training, and reporting unsafe conditions. Maintaining resident care information confidentiality is crucial. The role requires working in office and hospital areas, potentially beyond normal working hours, and communicating with medical staff, nursing, therapy, and administrative personnel. Assisting with in-house admissions as needed is also part of the responsibilities.

Requirements

  • Prior marketing and sales experience in the hospital, LTC/SNF, home health and/or hospice clinical setting.
  • Must possess clinical evaluation and case management skills, with expert knowledge of various insurances and reimbursements
  • Experience with electronic medical records and computer documentation systems.
  • Must be able to read, write, speak, and understand the English language
  • Ability to follow oral or written instructions
  • Must possess the ability to make independent decisions when circumstances warrant such action
  • Must possess the ability to deal tactfully with personnel, residents, visitors and the public
  • Must be calm and levelheaded in emergencies
  • Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing admission practices
  • Strong knowledge in the use of computers, data input/retrieval/output functions
  • Must have patience, tact, cheerful disposition and enthusiasm, handle residents, staff, and visitors, based on maturity level at which they are functioning

Responsibilities

  • Act as a liaison between healthcare providers to ensure continuity of care for resident transition from the hospital to home health
  • Effectively manage referrals into skilled nursing facilities
  • Perform qualifying assessments to determine the appropriateness of all resident referrals and begins the admission process at that time
  • Serve as a resource for discharge planners, patients and/or their families to educate them about our various services and programs
  • Responsible for meeting established referral/admissions goals as defined by Administrator
  • Demonstrate strong relationship and sales skills required to develop long-term relationships that effectively generate resident referrals and admissions
  • Deliver education and special presentations to hospital staff on skilled nursing facility services
  • Participate in community organizations that directly impact referral sources and resident population
  • Responsible for completing resident evaluations in a timely, accurate and effective manner while adhering to all policies and procedures
  • Regular, punctual attendance
  • Serve as a member of committees whose purpose is to enhance hospital relations
  • Attend sales meetings and assist in implementation of resulting plans; monitor and track results
  • Report known or suspected incidents of fraud, waste and or abuse to supervisor
  • Participate in all compliance training programs
  • Attend and participate in workshops, seminars, etc., as approved
  • Report all unsafe/hazardous conditions, defective equipment, etc., to supervisor immediately
  • Maintain the confidentiality of all resident care information including protected health information; report known or suspected incidents of unauthorized disclosure of such information
  • Work in office areas as well as throughout assigned Hospital areas
  • Work beyond normal working hours when necessary
  • Communicate with the medical staff, nursing and therapy personnel at the hospital as well as the Administrator, DNS and office staff
  • Maintain a liaison with the residents, their families, support personnel, etc., to ensure that the residents’ needs are continually met
  • Assist with in-house admissions process as needed.
