About the position
The Clinical Director (Registered Nurse) at Divine Purpose Care Residential Services is responsible for overseeing the daily clinical operations of the home health care team. This role focuses on ensuring quality healthcare is provided to patients in their homes, promoting independence, and enhancing overall well-being. The Clinical Director will lead a team of healthcare professionals, manage patient care plans, and ensure compliance with healthcare regulations.
Responsibilities
- Oversee the daily clinical operations of the agency.
- Supervise Home Health Aides (HHAs) and professional staff.
- Conduct patient assessments and develop individualized care plans.
- Administer prescribed medications and treatments.
- Monitor patient progress and update medical records.
- Educate patients and families on disease management and self-care techniques.
- Coordinate care with other healthcare professionals to ensure comprehensive patient treatment.
- Provide emotional support and guidance to patients and their families.
- Participate in meetings, including Governing Body, QAPI, and other leadership and non-leadership gatherings.
- Perform other duties as outlined in the job description.
Requirements
- Registered Nurse (RN) with prior home healthcare experience.
- Strong leadership and team management skills.
- Proficient in documentation and maintaining accurate patient records.
- Knowledge of physiology, anatomy, and disease processes.
- Familiarity with HIPAA regulations and patient confidentiality.
- Experience with Medicare/Medicaid regulations and budgeting in home healthcare.
